Please be prepared to provide 2 work references who we may contact to verify previous employment dates and performance. Minimal computer skills are required and or willingness to be trained.
Requirements for various departments:
Day Habilitation with Children and Adults – Working one on one with client instructing and increasing Independent Living Skills and Community Integration skills.
- Two employment references
- A copy of HS Diploma or GED
- State Police background check is required if hired
- A FBI check, if not a resident of the state for the past five years, is required if hired
Personal Care – In-home care for children and adults
- CNA license – please attach with application
- EVV Registration – Letter & EVV Number – please attach with application
Alternate Living (with Adults) – Twenty-four hour care in employees home.
- Three personal references
- A completed central registry check and criminal records check from all members of your household over 14 years of age
- A physical exam form filled out by your physician on all family members
- FBI background check if not a resident of the state for the past five years
Foster Parent – Foster parent applications will be given a mandatory one hour informal meeting.
Day Care – Instruction to help children overcome developmental delays.

All Applicants
After hire, all employees will be required to complete training for a minimum of 12 hours for Developmental Disabilities programs, 32 hours for foster parents, and 40 hours to obtain personal care certification. In addition, there will be on-going training updates required for all employees.
Auto insurance, driver’s license, and T.B. skin test must be brought to the office prior to your first day of Employment. Driver’s license, auto insurance, T.B. skin test must be kept current at all times. Integrity will administer T.B. skin tests Monday, Tuesday, Wednesday and Friday. Please call to schedule.
